COMMUNITY & FAMILY SUPPORT SERVICES

Community and Family Support Services provides Family Aide, Parent Aide, Counseling, Family Therapy, or other in-home/in-community services.  Services are appropriate to youth and families facing the difficulty of re-entry into the community following an episode of “out-of-home care” and families who are working to prevent separation of the family members.  The goal of this program array is to provide the needed service to assure safety and functionality in the community.  These services are available in Springfield (Greene and surrounding Counties), Moberly (Boone, Cole, Randolph and Callaway Counties), St. Louis metropolitan area and Farmington.

 

 

 

Admission Criteria

  • Services are a component of overall plan of care directed by a competent team (FST)
  •  Community-based services are appropriate to need

Appropriate for Service

  • Needing support on re-entry for “out-of-home” care 
  • Family members needing skill enhancement
  • Parents needing supports to maintain children in their own care
  • Children who will “act out” in the absence of extra attention or service
